• Serves as a liaison for the newly arrived refugees between HCPH Refugee Health Screening program and the Local Resettlement Agencies and/or service providers, social services, and other healthcare providers
• Monitors, tracks, schedule and follows up on abnormal test results/findings/missed appointments by telephone, postal correspondence, or home visits
• Ensures transportation and translation services are provided for the patients at their appointment
• Assists with patient screening (i.e. drawing blood, testing urine, taking/recording vital signs, heights and weights, vision and hearing assessment)
• Prepare and label samples (stools for parasite, blood, sputum, etc.) for transport to appropriate laboratories
• Provide in-room support for all clinical staff during the clinic visits as needed.
• Enters registration data into the electronic medical record system on all new refugee clients and scan medical records/chart for medical screening visit.
• Organizes transportation and interpretation services for patients to clinic appointment
• Conducts home visits when necessary to retrieve medical records, obtain consents and/or collect specimens.
• Assist in performing data entry and electronic data processing by using the program EMR, Microsoft Excel, and any other related software.
• Other duties as assigned.
