• Responsible for ensuring appropriate follow up with participants who require ongoing care, provide facilitated assistance to participants as they navigate the dental care system
• Utilize the oral health assessment results to inform participants about the dental services they need, create referrals for care and work to establish a Dental Home to promote ongoing care
• Perform dental registration and assist in the field with patient inquiries when assigned
• Schedule, confirm and follow up with participants regarding missed appointments to support schedule efficiency
• Connect clients to community resources and services for which they may be eligible including transportation services, financial assistance programs and language translation to facilitate appointments
• Work with other team members to monitor and create social media posts that promote dental education and dental related topics
• Provision of preventive dental services for any of the HCPH dental programs including the mobile dental unit, HCPH clinics, school-based events and/or other community settings when assigned
• Oral health promotion and demonstration of proper oral hygiene techniques in various community settings
• Other duties as assigned
